Below you can access the paperwork needed to run the HENRY Cooking for a Healthy Family programme.
Completed copies of items marked ** should be returned by facilitators to their local HENRY Coordinator at the end of each programme.

Cooking for a Healthy Family
1. Attendance register**
To be completed each week to record how many sessions each parent attends. Please provide the parents' ID codes and details of there referral pathway. Please add reasons for non-attendance and actions taken to follow up for each session.
2. Example contacts reference sheet
You may wish to keep a reference sheet of attendees' contact details, to help you stay in touch with parents between sessions. This is an example reference sheet you can use.
Please note: this information is confidential and should not be shared with anyone.
To protect parents' privacy, you must not return this document to HENRY.
3. Parent review forms - for sessions 1 to 5
For parents to complete each week at the end of the session. They should form part of facilitators’ reflective review each week and be used in supervision. Please do not return these to HENRY at the end of the programme. Best practice is to destroy them.
4. Parent baseline questionnaire**
Ask parents to complete this at or before the beginning of the first session. See our Top tips for introducing questionnaires sensitively and effectively.
Important: both baseline and completion questionnaires should be printed directly from this page – photocopies of questionnaires cannot be read by our scanning software when processing data for generating your termly and annual outcomes reports, meaning you will receive an incomplete report.
5. Parent completion questionnaire**
Ask parents to complete this at the last session. This questionnaire repeats the questions asked in the baseline questionnaire to allow evaluation of programme impact. See our Top tips for introducing questionnaires sensitively and effectively.
Please pair up matching baseline and completion questionnaires together for each parent.
6. Facilitator weekly review form - for Sessions 1 to 5
Please ensure that both facilitators complete a separate form at the end of each session, using it to help each other debrief together after the session.
7. Facilitator final review form**
A more detailed review reflecting on the programme as a whole. Each facilitator completes a separate form.
8. Programme certificate
To be printed on white card, completed, signed, and presented to parents at the end of the programme. Parents need to have attended at least 4 sessions to receive a certificate.
