Below are links to the online forms and copies of other paperwork needed to set up and run the HENRY online A Healthy Start in Childcare training.

• Register a training form 

Please complete this form before the training.  This will create the training ID that is used on all the review forms for this course.

• Trainer review form   

Please complete this form after sessions 1 and 3 of the training.  

• Participant review form

The online participant review form is completed during session 3: During the comfort break in session 3, please send participants an email with the review form link, the Area code and the Training ID.

• Submit a training 

Please complete this form within 3 days of the training.  You will be asked to record the number of participants who completed the training.  

• Slides to share

This zip folder contains three documents which are shared on-screen during this course.

• Participant Information

These documents includes the information that is sent to participants through Arlo prior to the training. The zoom details are sent as part of the reminders sent 14, 7 and 1 day prior to the start of the course
