Below are sample copies of the forms and other paperwork needed to set up and run HENRY In-Person Core Training. If possible please print the various items onto the colour of paper specified.
1. Order Resources
- Each participant will need a training resource set for this course – this includes the workbook, the Facilitator Manual for the programme, and the parent resources

2. Register a Training Course**
- It is important to register each course in advance by clicking this link, creating a unique Training ID Code

- Please send this to participants in advance of the training. You will need to include course details
5. HENRY A4 Poster
The A4 posters are to welcome trainees to the venue – to the building and to the training room.
6. HENRY A5 sign
The A5 sign is to fold and display on a welcome table. We recommend laminating it.
7. Attendance Register**
Please print 2 copies of this double-sided form, to allow enough space for everyone to sign in.
At the end of Day 2, please record whether participants have been authorised (A), conditionally authorised (C) or not authorised (N) in the Code column. Where participants have been conditionally or not authorised, record on the back of the attendance sheet the reasons for this and the agreed next steps. This attendance register needs to be completed for each course, and a copy sent to HENRY.
Towards the end of Day 1, each participant will be allocated a short skills practice topic to prepare. These topics will be delivered on Day 2.
Both trainers make notes about the delivery of each skills practice topic on the Practising Skills sheet – this will support your individual feedback with each participant. Please return a copy of these notes to HENRY
9. Participant Review Forms**
This online form is for participants to complete at the end of the training. It is designed to inform trainers about what has worked well/less well, it enables us to report on course outcomes and is part of the continuing quality assurance of HENRY training.

11. Certificates
To be printed on good quality white card, completed by referring to trainees’ names on the attendance sheet, signed by both trainers, and presented to trainees at the end of the course. Trainees need to have attended both days’ training to receive a certificate.
