In-Person Core Training

Below are sample copies of the forms and other paperwork needed to set up and run HENRY In-Person Core Training. If possible please print the various items onto the colour of paper specified.

Completed copies of items marked ** must be returned to HENRY after each training.

1. Order training resources

  • Each participant will need a training resource set for this course – this includes the workbook, the A Healthy Start practitioner handbook, and an envelope containing a number of other HENRY resources
  • Please order these to a single address, allowing 4 weeks for delivery
  • Please then distribute these to participants so they receive them 1 week prior to the course start so they can complete the pre-work
  • Email your order Please note, different resource ordering and delivery arrangements apply for Northern Ireland.

5. HENRY A4 Poster
The A4 posters are to welcome trainees to the venue – to the building and to the training room.

6. HENRY A5 sign
The A5 sign is to fold and display on a welcome table. We recommend laminating it.

7. Attendance Register**
Please print 2 copies of this to allow enough space for everyone to sign in. This needs to be completed for each course, and a copy sent to HENRY.

8. Group Facilitation Training flyer
To be made available on Day 2 of the course for those who are interested in undertaking further HENRY training and leading HENRY Healthy Families group programmes.

10. Training Review Forms**

Day 1

Please print copies of this form for training participants to complete at the end of Day 1.  It is designed to inform trainers about what is working well/less well from participants’ point of view. This form does not need to be returned to HENRY.  The form can be found here.

Day 2

This online form is for participants to complete at the end of the training. It is designed to inform trainers about what has worked well/less well, it enables us to report on course outcomes and is part of the continuing quality assurance of HENRY training.

11. Trainer Review Forms**

12. Certificates

To be printed on good quality white card, completed by referring to trainees’ names on the attendance sheet, signed by both trainers, and presented to trainees at the end of the course. Trainees need to have attended both days’ training to receive a certificate.
